Vitamin D3 dosing to reach a target serum 25(OH)D uses a simple rule: ~100 IU/day raises serum level by ~1 ng/mL at 70 kg body weight. The daily dose scales roughly with body weight.

For baseline deficiency (<20 ng/mL), an 8-week loading protocol (50,000 IU weekly) is the Endocrine Society standard, then drop to maintenance. Dosing above 2,000 IU/day should include vitamin K2 and magnesium cofactors.

Where the D3 evidence actually clusters

Raising 25(OH)D to a target range is where almost all the evidence for vitamin D3 clusters. Correcting deficiency (<20 ng/mL) produces the biggest downstream effects; hip-fracture reduction, fall prevention in the elderly, autoimmune-disease-incidence reduction (VITAL, HR 0.78), preeclampsia-risk reduction in pregnancy. Benefits in already-replete people are minimal.
